Terms & Conditions

These Terms & Conditions (“Terms”) govern your purchase and use of CareTap. By submitting payment, signing an order form, or using CareTap services, you agree to these Terms. This page is provided for informational purposes and is not legal advice.

1) Service Overview

CareTap provides a branded, mobile-friendly clinic page accessible via QR code, with configurable links (e.g., online booking, reviews, website, directions, intake forms, social media, and other helpful URLs). CareTap is billed annually.

2) Deliverables & Scope

  • Initial setup of your CareTap page (logo, colors, links/buttons, optional custom HTML/media section).
  • QR code artwork file(s) and a high-resolution QR file for your own uses.
  • Updates/edits are included. Additional work outside normal CareTap scope may be quoted separately.

3) Billing & Renewal (Annual)

  • CareTap is billed annually in advance (currently $499/year unless otherwise agreed in writing).
  • Your plan may auto-renew each year unless cancelled prior to the renewal date.
  • If a payment fails, we may suspend access (including analytics) until payment is resolved.

4) Satisfaction Edits & Refunds

  • All sales are final. We do not offer refunds once setup or design work has begun or access has been provisioned.
  • Not satisfied? We will make reasonable edits to your CareTap page at no extra cost until it aligns with what you want, within normal CareTap use.

5) Acceptable Use & Review Platform Compliance

  • You agree to use CareTap in compliance with applicable laws and the terms of the platforms you link to (e.g., Google, Yelp, Facebook, Instagram, TikTok). Do not solicit reviews in a manner that violates those platforms’ policies (e.g., offering incentives, gating, or review filtering).
  • You’re responsible for the accuracy and legality of all content and links you provide.

6) Content & Ownership

  • You retain ownership of your logos, brand assets, and copy you supply. You grant CareTap a non-exclusive license to use those assets to provide the service.
  • CareTap retains ownership of its software, templates, and system designs.

7) Data & Privacy

  • CareTap collects basic analytics (e.g., page views and button clicks) to provide reporting. We do not sell your data.
  • Do not submit Protected Health Information (PHI) through CareTap. CareTap is not a medical records system and is not intended to store PHI.

8) Availability & Changes

  • We aim for high availability, but occasional maintenance, provider outages, or changes to third-party platforms may affect features or uptime.
  • We may update features, pricing, or these Terms. Material changes will be posted on this page with an updated date.

9) Disclaimers

CareTap is provided “as is” without warranties of any kind, express or implied. We do not guarantee any specific business outcomes (e.g., a specific number of reviews, followers, or bookings).

10) Limitation of Liability

To the maximum extent permitted by law, CareTap’s total liability for any claims arising out of or related to the service is limited to the amount paid by you to CareTap for the applicable one-year term. We are not liable for indirect, incidental, special, consequential, or punitive damages.

11) Termination

You may cancel before renewal to avoid being charged for the next term. We may suspend or terminate service for violation of these Terms or unlawful use. No refunds are issued for early termination.

13) Acceptance

By submitting payment or using the service, you confirm that you’ve read and agree to these Terms. If purchasing via a hosted order form (e.g., Jotform + Stripe), that submission constitutes acceptance.

If your legal team needs a signed copy or an addendum (e.g., specific procurement terms), reach out and we’ll coordinate.
